About the Job
Join a compassionate, patient-centered team at CenterWell Senior Primary Care, a leading provider of value-based care for seniors. As part of Humana’s Primary Care Organization, CenterWell operates 340+ centers across 15 states, offering a team-based approach to wellness through CenterWell and Conviva brands. Our physicians lead interdisciplinary care teams dedicated to treating the whole person—physically, emotionally, and socially.
Location: 4813 S. Eastern Avenue, Las Vegas, NV
Compensation: $203,400 – $299,500 base pay (plus quality bonuses, potential to exceed $300K)
Schedule: Monday to Friday, 8:00 a.m. – 5:00 p.m.

Key Responsibilities

Deliver direct patient care with a focus on preventive, senior-focused medicine.

Lead a multi-disciplinary care team through daily clinical huddles.

Manage patient care coordination including referrals, hospital and SNF transitions, and DME/home health services.

Ensure timely and accurate documentation and collaborate with quality coders.

Participate in call rotation, growth initiatives, and compliance efforts.
Qualifications
Required:
-
MD or DO from an accredited institution
-
Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatrics
-
Active or eligible medical license in Nevada
-
Passion for value-based care and team-based models
-
Strong communication, organizational, and clinical documentation skills

Basic computer proficiency and EMR experience
Preferred:

Active DEA license
-
Medicare/Medicaid provider credentials
-
2–5 years of clinical experience with Medicare Advantage patients

Bilingual a plus
