About the Role:
Join Humana’s Primary Care Organization, a leading senior-focused, value-based care provider operating across eight states. As a Primary Care Physician (PCP), you will lead a multidisciplinary care team delivering comprehensive, patient-centered care designed to improve outcomes for seniors. Our model emphasizes quality, coordinated care supported by analytics, allowing PCPs to focus on fewer patients with more meaningful engagement.
Key Responsibilities:

Provide high-quality evaluation and treatment aligned with best practices.

Coordinate patient services, including referrals, hospital transitions, and home care.

Actively collaborate with care teams through daily huddles and continuous communication.

Maintain timely, accurate medical documentation in partnership with coding specialists.

Participate in quality improvement, growth initiatives, and local on-call programs.

Ensure compliance with licensing, certification, and organizational policies.

Dedicate 100% of your time to clinical patient care, including direct and administrative tasks related to patient care.
Qualifications:

MD or DO from an accredited program with current, unrestricted medical license.

Strong communication skills and commitment to integrated, team-based care.

Adaptability to value-based care environments.

Preferred: Board certification in Family Medicine, Internal Medicine, or Geriatrics; DEA license; Medicare provider number; 2-5 years clinical experience managing Medicare Advantage patients; knowledge of Medicare guidelines and HEDIS quality measures.
Compensation & Benefits:

Competitive base salary ($203,400 – $299,500) plus bi-annual incentives.
