Each location features a multidisciplinary Care Team composed of physicians, care coaches, behavioral health specialists, clinical pharmacists, and other healthcare professionals, providing holistic care around the clock.

Responsibilities

Provide comprehensive evaluation and treatment of patients at the center, adhering strictly to established standards of care.

Monitor and ensure the quality of medical care delivered, utilizing available data and chart reviews to track patient outcomes.

Assist in coordinating a range of patient services, including specialty referrals, hospital and skilled nursing facility (SNF) coordination, durable medical equipment provisioning, and home healthcare services.

Serve as an integral member of the interdisciplinary care team, contributing medical expertise during daily team huddles.

Support the Regional Medical Director and Center Administrator by fostering a cooperative and professional atmosphere within the practice through approachable and collaborative behavior.

Complete all required medical documentation promptly and accurately, collaborating with quality-based coders to enhance coding precision.

Comply with policies and protocols established by Clinical Leadership.

Participate in regular meetings with the Regional Medical Director to review quality of care, analyze outcome data, discuss policies, procedures, and address any record-keeping concerns.

Engage in identifying and pursuing opportunities for growth by expanding existing services or introducing new ones within the center.

Contribute to the local primary care on-call rotation as needed to support CenterWell operations.

Maintain full compliance with all licensing, certification, and accreditation requirements.

Dedicate 100% of working hours to direct patient care, including face-to-face patient interactions and related administrative tasks such as charting and meetings focused on patient care.
Required Qualifications

Graduation from an accredited Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) program.

Board certification or eligibility for certification by the American Board of Medical Specialties (ABMS) or the American Osteopathic Association (AOA) in Family Medicine, Internal Medicine, or Geriatric Medicine.

Current, unrestricted medical license in the state of practice or willingness to obtain one; eligibility and willingness to obtain licenses in other states within the assignment region, if required.

Strong interpersonal skills, capable of effective communication and relationship-building with both colleagues and patients.

Basic proficiency with computer applications, including email and Electronic Medical Records (EMR).

This is a patient-facing role that requires participation in a Tuberculosis (TB) screening program; successful candidates must undergo TB screening.
Preferred Qualifications

Active and unrestricted DEA (Drug Enforcement Administration) license.

Medicare Provider Number.

Medicaid Provider Number.

Two to five years of relevant clinical experience is preferred.

Experience managing a Medicare Advantage patient panel with a strong understanding of best practices in coordinated, value-based care settings.

Knowledge of Medicare guidelines and coverage policies.

Familiarity with Healthcare Effectiveness Data and Information Set (HEDIS) quality indicators.

Bilingual abilities are an advantage.

Language Proficiency Testing: Any associate communicating with patients in a language other than English must complete a language proficiency assessment, administered by an external provider, to ensure competency.
