About the Job: Pioneering Care for Complex Patients within NYC Correctional Health Services
NYC Health + Hospitals/Correctional Health Services (CHS), the direct healthcare provider within NYC jails since 2016, is seeking dedicated Physicians to join their innovative Outposted Therapeutic Housing Units (OTxHU) program. This groundbreaking initiative, located within three NYC Health + Hospitals acute care facilities, aims to bridge the gap between in-jail care and inpatient hospitalization for incarcerated individuals with complex health conditions. As a key member of an interdisciplinary team, you will provide comprehensive and compassionate primary care, including diagnoses, treatment of acute and chronic illnesses, and consultation assessments.
Responsibilities include:
- Managing acute and chronic conditions, including ordering and interpreting tests and medications.
- Conducting thorough histories and physical examinations.
- Collaborating with Physician Assistants and other specialists.
- Ensuring adherence to quality care standards and implementing consultant recommendations.
- Facilitating transitions of care and addressing patients’ special needs.
- Educating patients on their conditions and treatment options.
- Maintaining accurate and timely documentation.
- Responding effectively to medical emergencies.
- Participating in quality improvement initiatives and maintaining clinical competency.
Ideal candidates will possess:
- Board certification/eligibility in Internal Medicine, Family Medicine, or a related primary care specialty.
- 3-5 years of relevant experience, including residency.
- Experience in skilled nursing or residential settings, with patients experiencing serious mental illness and/or substance use disorders (harm reduction approach preferred).
- Familiarity with trauma-informed care and shared decision-making.
- Strong communication and collaboration skills.
- New York State medical license.
