Job Summary
We are looking for an experienced Primary Care Mental Health Practitioner to join the 4Doncaster Primary Care Network (PCN). This role is an exciting opportunity to work at the heart of primary care, providing direct mental health support to patients while enhancing collaboration between primary and secondary care services. You will be a key point of contact for GPs, offering specialist advice and ensuring seamless access to mental health care.
Key Responsibilities

Deliver direct mental health support and interventions to patients within primary care settings.

Work alongside GPs and other healthcare professionals to assess, triage, and manage mental health concerns.

Provide expert guidance on referrals and support pathways between primary and secondary care services.

Conduct bio-psycho-social assessments and develop tailored treatment plans.

Participate in multi-disciplinary meetings, including safeguarding and risk management discussions.

Support service transformation initiatives to enhance mental health care across the PCN.

Contribute to the training and development of primary care staff in mental health awareness.
Requirements
Essential:

Professional qualification in mental health nursing, social work, or a related field.

Experience in assessing and managing mental health conditions in primary or community settings.

Ability to provide clinical advice and support to primary care teams.

Strong communication and multi-disciplinary collaboration skills.
Desirable:

Experience working in a PCN or integrated healthcare system.

Knowledge of NHS mental health pathways and referral processes.

Experience in service development and mental health strategy implementation.
