Responsibilities

Evaluate and treat patients in line with care standards.

Coordinate patient services, including referrals and care coordination.

Participate in team-based care, contributing to daily huddles and fostering collaboration.

Document medical records in a timely manner and optimize coding accuracy.

Support quality improvement initiatives and patient outcome reviews.

Provide direct patient care and ensure compliance with licensing and certification requirements.
Required Qualifications

MD or DO with board certification or eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine.

Active, unrestricted medical license or willingness to obtain it.

2-5 years of relevant experience preferred.

Strong communication skills and ability to work in an integrated team.

Basic computer proficiency, including email and EMR systems.

Bilingual English/Spanish (preferred).

Competitive salary range: $203,400 – $299,500/year, plus quarterly bonus opportunities.
