Responsibilities

  • Evaluate and treat center patients according to established standards of care.

  • Monitor and ensure the quality of care for patients, using available data and chart reviews to track progress.

  • Assist in coordinating patient services, including specialty referrals, hospital and skilled nursing facility (SNF) coordination, durable medical equipment (DME), and home health care.

  • Actively participate as a key medical expert in daily huddles with the care team.

  • Collaborate with the Regional Medical Director and Center Administrator to foster a professional, cooperative environment through effective communication.

  • Complete all medical documentation in a timely manner, working with quality-focused coders to ensure accurate coding.

  • Adhere to clinical policies and protocols set by Clinical Leadership.

  • Participate in meetings with the Regional Medical Director to review care quality, outcomes, policies, and procedures.

  • Contribute to the growth of new or existing services within the Center.

  • Participate in the local primary care “on-call” program as needed.

  • Ensure compliance with licensing, certification, and accreditation requirements.

  • Dedicate 100% of time to direct patient care, including patient-facing and administrative tasks (e.g., charting and meetings).

Required Qualifications

  • Graduate from an accredited MD or DO program.

  • Board Certified or eligible for certification (ABMS or AOA) in Family Medicine, Internal Medicine, or Geriatric Medicine.

  • Current, unrestricted medical license or willingness to obtain state licensure; eligibility to obtain licenses in other states as needed.

  • Minimum of 2-5 years of relevant experience preferred.

  • Strong verbal and written communication skills.

  • Ability to maintain positive relationships with colleagues and patients.

  • Commitment to the “Integrated Team-Based Care” model.

  • Willingness to adapt to and learn within a value-based care setting.

  • Superior patient/customer service skills.

  • Basic computer skills, including email and electronic medical records (EMR).

  • This role is patient-facing and requires participation in the Tuberculosis (TB) screening program. TB screening will be required for candidates selected for this role.

Preferred Qualifications

  • Active, unrestricted DEA license.

  • Medicare and Medicaid Provider Numbers.

  • Experience managing Medicare Advantage panels, with knowledge of best practices in coordinated care and value-based care environments.

  • Knowledge of Medicare guidelines and coverage.

  • Familiarity with HEDIS quality indicators.

  • Bilingual in English and Spanish, with the ability to speak, read, and write in both languages without limitations.

Language Proficiency Testing

  • Any partner associate interacting with a member in a language other than English must undergo a language proficiency assessment. The Interagency Language Rating (ILR) test, provided by the Federal Government, will be required for applicants.

Schedule & Compensation

  • Monday to Friday: 8:00 a.m. to 5:00 p.m.

  • Scheduled Weekly Hours: 40

  • Pay Range: $203,400 – $299,500 annually (compensation will vary based on geographic location, skills, experience, education, and certifications.
