Join CenterWell Senior Primary Care and dedicate your skills to providing comprehensive, patient-centered care to seniors. Our integrated, team-based model allows you to focus on the holistic well-being of your patients, spending quality time to understand and address their unique needs.
Your Responsibilities:
- Evaluate and treat CenterWell patients according to the highest standards of care.
- Ensure and monitor the quality of patient care through data analysis and chart reviews.
- Complete timely and accurate medical record documentation, collaborating with quality-focused coders.
- Coordinate comprehensive patient services, including referrals, hospital and SNF coordination, DME, and home healthcare.
- Actively participate in daily center huddles.
- Maintain a professional and approachable manner with patients and colleagues.
- Adhere to clinical policies and protocols.
- Collaborate with medical directors on quality initiatives, data review, and procedural matters.
- Engage in potential growth opportunities for center services.
- Participate in the local primary care on-call program as needed.
- Maintain personal compliance with all licensing, certification, and accrediting requirements.
- Dedicate 100% of your time to direct patient care, encompassing both patient-facing interactions and related administrative tasks.
What We’re Looking For:
- Graduate of an accredited MD or DO program with successful completion of an accredited ACGME/AOA residency.
- Board Certification or eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine (ABMS or AOA).
- Current and unrestricted medical license or the ability to obtain one in the state of practice, with willingness to secure additional state licenses as needed.
- Excellent verbal and written communication skills.
- Strong interpersonal and communication skills with colleagues and patients.
- Full engagement with the integrated team-based care model.
- Eagerness to learn and adapt in a value-based care environment.
- Exceptional patient/customer service skills.
- Basic computer proficiency, including email and EMR.
- Willingness to undergo Tuberculosis (TB) screening due to the patient-facing nature of this role.
Preferred Qualifications:
- Active and unrestricted DEA license.
- Medicare & Medicaid Provider Number.
- Bilingual proficiency in English and Vietnamese (speaking, reading, and writing without limitations).
