Job Summary
The VA Illiana Health Care System is seeking full-time Family Practice or Internal Medicine physicians for its Community-Based Primary Care Clinic in Peoria. This role involves providing comprehensive care under the Patient Aligned Care Team (PACT) model, focusing on preventive care, patient education, and high-quality primary care. A Recruitment/Relocation Incentive is available for highly qualified candidates.
Key Responsibilities

Manage chronic conditions and treat inter-current medical issues

Collaborate with the PACT to provide coordinated, continuous care

Perform screenings and health promotion activities

Address common mental health disorders and make referrals as needed

Supervise and collaborate with mid-level primary care providers

Participate in peer reviews and attend staff meetings
-
Complete timely documentation in line with facility policy
Work Schedule
Monday-Friday, 7:45 a.m. – 4:15 p.m.

Qualifications

U.S. Citizenship
-
Doctor of Medicine or equivalent degree from an accredited school

Full, unrestricted license to practice in the U.S.

Completed residency training leading to board certification eligibility

Proficiency in English
Preferred Experience

Board Certified in Family Medicine or Internal Medicine
Physical Requirements

Light lifting (under 15 lbs), standing, walking, and emotional stability required
