Job Summary
The VA Illiana Health Care System is seeking full-time Family Practice or Internal Medicine physicians for its Community-Based Primary Care Clinic in Peoria. This role involves providing comprehensive care under the Patient Aligned Care Team (PACT) model, focusing on preventive care, patient education, and high-quality primary care. A Recruitment/Relocation Incentive is available for highly qualified candidates.

Key Responsibilities

  • Manage chronic conditions and treat inter-current medical issues

  • Collaborate with the PACT to provide coordinated, continuous care

  • Perform screenings and health promotion activities

  • Address common mental health disorders and make referrals as needed

  • Supervise and collaborate with mid-level primary care providers

  • Participate in peer reviews and attend staff meetings

  • Complete timely documentation in line with facility policy

Work Schedule
Monday-Friday, 7:45 a.m. – 4:15 p.m.

Qualifications

  • U.S. Citizenship

  • Doctor of Medicine or equivalent degree from an accredited school

  • Full, unrestricted license to practice in the U.S.

  • Completed residency training leading to board certification eligibility

  • Proficiency in English

Preferred Experience

  • Board Certified in Family Medicine or Internal Medicine

Physical Requirements

  • Light lifting (under 15 lbs), standing, walking, and emotional stability required
