About the Role
CenterWell Senior Primary Care, a Humana company, is seeking a Primary Care Physician (PCP) to lead in our value-based, team-oriented care model. Focused on improving health outcomes for seniors, our integrated care teams provide personalized, concierge-style care supported by advanced analytics, coordinated services, and deep community engagement. With fewer patients and longer visits, you’ll have the time to deliver exceptional care.
Key Responsibilities

Deliver comprehensive primary care aligned with clinical standards and protocols

Collaborate in a team-based care model (MA, NP/PA, behavioral health, pharmacists, etc.)

Participate in daily care huddles and coordinate specialty referrals and post-acute care

Document accurately and timely with support from quality coders

Ensure patient care meets quality, compliance, and value-based care guidelines

Spend 100% of your time on direct patient care, including admin tasks related to patient care
Qualifications
Required:

MD or DO from an accredited program

Unrestricted medical license (or ability to obtain) in practicing state

Current DEA license (or willing to obtain)

Strong communication and teamwork skills

Comfortable practicing in a value-based care model

Basic computer/EMR proficiency

TB screening required (patient-facing role)
