About the Role
CenterWell Senior Primary Care, a Humana company, is seeking a Primary Care Physician (PCP) to lead in our value-based, team-oriented care model. Focused on improving health outcomes for seniors, our integrated care teams provide personalized, concierge-style care supported by advanced analytics, coordinated services, and deep community engagement. With fewer patients and longer visits, you’ll have the time to deliver exceptional care.


Key Responsibilities

  • Deliver comprehensive primary care aligned with clinical standards and protocols

  • Collaborate in a team-based care model (MA, NP/PA, behavioral health, pharmacists, etc.)

  • Participate in daily care huddles and coordinate specialty referrals and post-acute care

  • Document accurately and timely with support from quality coders

  • Ensure patient care meets quality, compliance, and value-based care guidelines

  • Spend 100% of your time on direct patient care, including admin tasks related to patient care


Qualifications

Required:

  • MD or DO from an accredited program

  • Unrestricted medical license (or ability to obtain) in practicing state

  • Current DEA license (or willing to obtain)

  • Strong communication and teamwork skills

  • Comfortable practicing in a value-based care model

  • Basic computer/EMR proficiency

  • TB screening required (patient-facing role)
