Key Responsibilities
-
Fill and label prescriptions accurately for inpatients, outpatients, and hospital staff.

Provide drug-related education and clinical guidance to patients, medical staff, and employees.

Dispense controlled substances while maintaining accurate inventory and compliance records.

Supervise and mentor pharmacy technicians, students, and residents.

Review medication profiles for safety, accuracy, and therapeutic effectiveness.

Prepare and schedule IV solutions, additives, and sterile compounds per USP standards.

Perform clinical interventions such as:

Medication profile review
-
IV-to-oral conversions
-
Renal dosing adjustments
-
Antimicrobial stewardship
-
Therapeutic drug monitoring (vancomycin, aminoglycosides, etc.)

Patient counseling and medication history collection
-
Participation in multidisciplinary rounds

Support process improvement, Medication Use Evaluation, and error prevention programs.

Ensure compliance with state/federal pharmacy laws, hospital policies, and safety standards.
Minimum Qualifications
Education:
-
Graduate of an accredited School of Pharmacy.
Licenses & Certifications:

Active Texas Registered Pharmacist license.

Pharmacy Sterile Product certification (through an accredited program or in-house training) required for sterile compounding.
Professional Growth & Development

Participate in continuing education and maintain licensure requirements.

Serve as a preceptor, mentor, and resource for new pharmacists, students, and residents.
