Duties Description

Monitor and Support Individual Health Needs:

  • Conduct weekly visits (at minimum) to assigned homes to provide nursing oversight and review health-related concerns.

  • Collaborate with primary care providers, specialists, and clinical teams as needed.

  • Advocate for individuals’ health care needs, ensuring they receive appropriate medical attention.

  • Maintain clear communication with the medical liaison or designated contact.

  • Support residential staff in delivering quality health services to individuals.

  • Follow proper reporting procedures and the chain of command to address individual or staff issues.

  • Complete and document follow-up care after emergency room visits or hospital stays.

  • Review, comment on, and sign off on lab results, x-rays, and specialist consultations, following up with practitioners when necessary.

  • Submit timely documentation of home visits.

  • Adhere to pressure wound care policies and documentation standards.

Deliver Nursing Services and Staff Support:

  • Respond promptly to emergency requests (both within and outside your assigned caseload).

  • Conduct nursing assessments and maintain comprehensive documentation, such as RN notes and nursing logs, within required timeframes.

  • Provide individualized nursing interventions.

  • Address staff inquiries related to medical and nursing concerns.

  • Ensure Plans of Nursing Services (PONS) are current, signed by staff, and electronically maintained.

  • Deliver health-related training and consultations to day programs, IES, and residential staff.

  • Conduct annual reviews of the medical sections of ISPs and medication audits for each home in your caseload.

  • Stay informed of changes in individuals’ medical sections within ISPs.

  • Obtain informed consent for medications in accordance with BDDSOO Policy.

Oversee Staff Competency in Delegated Nursing Procedures:

  • Conduct initial medication certifications and annual clinical performance evaluations of Direct Support Assistants (DSAs) for delegated nursing tasks, including medication administration.

  • Provide training on new or updated medication policies and procedures.

  • Submit certification and recertification records to CMHN, NA, and NPC staff in a timely manner.

  • Complete documentation and corrective actions for medication errors.

Provide Clinical Supervision to Licensed Practical Nurses (LPNs):

  • Offer nursing supervision and conduct performance evaluations (as assigned) on individual-specific nursing tasks.

Maintain Professional Standards and Development:

  • Participate in nursing discipline meetings and complete all mandated online DDSO and discipline-specific training.

  • Identify personal and departmental areas for improvement and contribute to action plans.

  • Undertake special projects and assignments as required.


Minimum Qualifications

  • Must possess a valid license and current registration as a Registered Professional Nurse (RN) in New York State.
