We are continuously hiring for direct support roles and frequently seek professionals in various clinical fields, including nursing, medicine, dentistry, psychiatry, psychology, social work, dietetics, and occupational therapy. Additionally, we offer numerous opportunities in administrative and clerical roles, as well as positions in health information management, facility maintenance, and operations.

Responsibilities

Monitoring and Oversight of Health Services

  • Assess and monitor the medical and health needs of individuals.

  • Conduct weekly visits to assigned residences to provide nursing oversight and complete necessary reviews.

  • Collaborate with primary care physicians, specialists, and clinical staff to ensure comprehensive care.

  • Advocate for individuals’ healthcare needs and ensure timely medical interventions.

  • Maintain ongoing communication with medical liaisons or designated personnel.

  • Support residential staff in providing consistent and high-quality health services.

  • Adhere to proper reporting mechanisms and follow the chain of command for medical or staff concerns.

  • Document and follow up on emergency room and hospital visits to ensure proper care.

  • Review and acknowledge laboratory results, X-rays, and specialist consultations, following up with providers as needed.

  • Complete and submit home visit documentation within required timeframes.

  • Follow pressure wound care protocols and ensure proper documentation.

Providing Nursing Support and Training

  • Respond to emergency medical requests promptly.

  • Conduct nursing assessments and complete required documentation, such as RN notes and nursing logs.

  • Administer individual-specific nursing interventions as required.

  • Address staff inquiries regarding medical and nursing concerns.

  • Maintain up-to-date Plans of Nursing Services (PONS), ensuring proper documentation and staff signatures.

  • Provide health-related training and consultations for day programs, residential staff, and other service areas.

  • Perform annual medical reviews and medication audits for individuals under care.

  • Stay informed about updates to individuals’ Individualized Service Plans (ISP) and medical records.

  • Obtain informed consent for medications in accordance with BDDSOO policy.

Supervising Medication Administration and Staff Competency

  • Conduct initial medication certification and annual clinical performance evaluations for Direct Support Assistants (DSAs) regarding nursing-delegated procedures.

  • Provide staff training on medication policies and procedures.

  • Ensure timely submission of certification and recertification records to appropriate databases.

  • Document and address medication errors, implementing corrective actions as necessary.

Supervising Licensed Practical Nurses (LPNs)

  • Provide clinical supervision and complete performance evaluations for LPNs regarding individual-specific nursing procedures.

Maintaining Professional Skills and Compliance

  • Participate in Nursing Discipline meetings and mandatory training sessions.

  • Complete online and in-person training to stay up to date with policies and medical advancements.

  • Identify opportunities for improvement in nursing practices and contribute to action plans.

  • Undertake special projects or assignments as needed.


Minimum Qualifications

  • A valid Registered Nurse (RN) license and current registration in New York State.

Employment under NY HELPS Program

  • This position is part of the New York Hiring for Emergency Limited Placement Statewide (NY HELPS) Program, which allows for non-competitive appointments (no exam required).

  • After one year, employees hired under NY HELPS are expected to transition to permanent competitive status without requiring an exam.

Salary & Pay Structure

  • Starting Salary: $93,929 per year (includes base salary, increased hiring rate, and geographic pay differential).

  • Job Rate: Up to $111,584 per year.

Additional Employment Details

  • Current New York State employees in eligible competitive positions may transfer under Section 70.1 of the Civil Service Law.

  • OPWDD employees affected by closures are given preference in hiring.
