Job Overview:
As a Registered Nurse (RN), you will be responsible for administering skilled nursing care to patients in their homes. You will coordinate care with the interdisciplinary team, patient/family, and referring agency while ensuring the overall coordination of patient care.
Key Responsibilities:

Patient Assessments:
Assess home patients to identify their physical, psycho-social, and environmental needs, ensuring comprehensive documentation, clinical record updates, and evaluation participation. -
Care Plan Implementation:
Develop, document, and implement personalized care plans to ensure quality and continuity of care for patients. -
Infection Control:
Deliver care in adherence to infection control measures that safeguard both patients and staff. -
Supervision and Clinical Direction:
Supervise and provide clinical guidance to home health aides and LVNs (Licensed Vocational Nurses) to ensure the quality and continuity of services. -
Effective Communication:
Maintain open lines of communication with patients/families, team members, and other healthcare professionals to ensure cohesive care delivery.
Qualifications:

Current Texas State Registered Nurse (RN) license.

Current CPR Certification.

Strong attention to detail and effective communication skills.

Minimum of 1 year of home health experience preferred.
