About the Role
Make a meaningful impact every day as a Home Health Registered Nurse (RN) with CenterWell. In this role, you’ll provide personalized, one-on-one care that helps patients recover, regain independence, and improve their quality of life — all in the comfort of their homes. You’ll collaborate with physicians, therapists, and other clinicians to create and manage individualized care plans, ensuring each patient achieves the best possible outcome.
Key Responsibilities

Conduct admissions, case management, and follow-up nursing visits for home health patients.

Provide hands-on patient care, implement treatment plans, and adjust as needed in consultation with physicians.

Collaborate with multidisciplinary teams, including PTs, OTs, aides, and external providers.

Supervise and guide Home Health Aides and Personal Care Workers, ensuring quality and documentation compliance.

Report patient condition, progress, and changes to physicians and Clinical Managers.

Prepare accurate clinical documentation including case conferences, updates, medication changes, and discharge planning.

Support patients and families in achieving recovery goals through teaching and care coordination.
Qualifications
Required:

Diploma, Associate, or Bachelor’s Degree in Nursing.

Current and unrestricted RN license.

Current CPR certification.

Valid driver’s license, auto insurance, and reliable transportation.

At least 1 year of nursing experience (med-surg, ICU, ER, or acute care preferred).
Preferred:

Previous Home Health nursing experience.

Strong organizational and communication skills.
Pay & Schedule
-
Base Pay: $70,500 – $96,900 per year.
-
Per Visit/Unit Pay: $45 – $63.

Hours: 40 hours per week (full-time).

Pay varies by location, skills, certifications, and experience.
