About the Role

We’re seeking a dedicated RN Case Manager to join our mission-driven team. In this role, you’ll provide personalized care that supports patients physically, emotionally, and spiritually while guiding their families with empathy and respect.
What You’ll Do
As an RN Case Manager, you will:

Develop and implement individualized care plans based on patient history and assessments.

Provide one-on-one nursing care, ensuring dignity and comfort at every stage.

Monitor vital signs and administer physician-ordered medications/treatments.

Educate patients and families on symptom management and care techniques.

Document patient progress and maintain accurate clinical records.

Communicate changes in condition and coordinate care with interdisciplinary teams.

Support patients and families with compassion, advocacy, and understanding.
What You’ll Bring

Valid Registered Nurse (RN) license in good standing (state of practice).

Current CPR certification.

Valid driver’s license and reliable transportation.

Strong communication, observation, and documentation skills.
