About the Role
We are seeking a Registered Nurse (RN) who is dedicated to delivering exceptional care and helping clients achieve the highest quality of life. In this role, you will assess health needs, create and implement nursing care plans, manage medical records, provide case management, and oversee clinical care delivery.

Key Responsibilities

Assess health problems and needs of individuals served and develop nursing care plans.

Administer nursing care and treatments while maintaining accurate medical records.

Provide case management and guidance on disease prevention and health maintenance.

Monitor staff performance in medication administration, meals, ADLs, and care plans.

Review incident reports, appointment findings, and implement changes as needed.

Approve prescribed medications/treatments and ensure accuracy of MARs.

Perform PRN assessments (falls risk, skin integrity, choking, etc.).

Oversee medication storage, narcotics counts, and timely ordering of prescriptions.

Complete monthly nursing summaries, reconciliations, and med exchanges.

Provide training, orientation, and on-call service as assigned.

Attend IPP meetings, submit assessments, and collaborate with interdisciplinary teams.
Qualifications
-
Degree from an accredited school of nursing OR current Registered Nurse (RN) license, in good standing.

Minimum of 1 year nursing experience (supervisory experience preferred).

Valid driver’s license and at least 18 years of age.

Strong organizational, communication, and leadership skills.

Ability to travel minimally to assigned operations/locations.
