Geisinger is seeking a compassionate Registered Nurse Home Visitor to join our Nurse-Family Partnership program. In this community-based role, you’ll provide guidance, education, and support to women and families through pregnancy, infancy, and early childhood. You’ll help families establish goals, access resources, and achieve healthier outcomes.

Key Responsibilities

Provide case management and home visitation using Nurse-Family Partnership protocols

Assess the physical, emotional, social, and environmental needs of families

Develop individualized visitation plans and evaluate outcomes

Educate and support mothers and families on pregnancy, infant, and toddler care

Deliver telehealth services in line with program guidelines

Build strong community relationships and promote the program to referral sources

Maintain timely, accurate documentation
Qualifications

Graduate of an accredited nursing program (BSN preferred)

Current PA RN license & BLS certification required
-
2+ years of nursing experience preferred (Maternal/Child Health, Pediatrics, Public Health, Home Visiting, or Behavioral Health a plus)

Must reside in or near Juniata or Mifflin County for work-life balance and accessibility
