Teladoc Health, a global leader in whole-person virtual care, is seeking a Nurse, Care Coordinator to provide proactive utilization management and care coordination services to members with high-risk needs, complex medication requirements, or inappropriate urgent/emergent care utilization. This role ensures high-quality, cost-effective healthcare coordination while delivering exceptional service.

Responsibilities

  • Deliver outstanding service to members, providers, and care teams via phone, video, and written communication.

  • Review member cases with external health plan risk teams, offering advice and support.

  • Collaborate with patient navigators and physicians to review and update care plans.

  • Maintain accurate documentation in care coordination systems.

  • Process referrals and prior authorizations promptly and accurately.

  • Conduct intake assessments, treatment planning, and reassessments to ensure coordinated, high-value, in-network care.

  • Evaluate clinical data to identify high-risk members and create action-oriented health goals.

  • Assist with medication reconciliation and post-hospitalization care navigation.

Required Qualifications

  • Active compact RN or NP license in good standing (ability to obtain licensure in all 50 states).

  • BSN or MSN required.

  • 1–3 years of clinical experience.

  • Strong customer service background in healthcare.

  • Excellent assessment, analytical, and communication skills.

  • Strong organizational skills and computer literacy.

Preferred Qualifications

  • Certified Case Manager (CCM) certification highly preferred.

  • Previous care coordination or virtual care experience.

  • Bilingual (English/Spanish) a plus.
