As a Home Health Registered Nurse with CenterWell Home Health, you’ll make a meaningful difference daily by delivering personalized, one-on-one care that supports patients’ independence in their own homes. Collaborating closely with a team of physicians and clinicians, you will develop and manage individualized care plans that promote recovery and help patients return to the life they cherish.
Key Responsibilities

Conduct admission assessments, case management, and follow-up skilled nursing visits for home health patients.

Provide ongoing care and manage each patient’s case, following guidance from the Clinical Manager.

Collaborate with physicians to develop initial treatment plans based on orders and patient assessments. Deliver hands-on care, monitor progress, educate patients, and revise care plans as needed in consultation with physicians and Clinical Manager.

Coordinate care across healthcare team members, including Physical Therapists, Occupational Therapists, Home Health Aides, and external providers.

Continuously communicate patient status, progress, and care updates to the physician and Clinical Manager.

Work with patients and families to implement care plans aimed at resolving health needs and improving outcomes.

Oversee and supervise Home Health Aides, Certified Home Health Aides, and Personal Care Workers; provide written care instructions aligned with the care plan and ensure proper documentation.

Facilitate patient discharge after consulting with physicians and Clinical Manager, completing all necessary clinical paperwork.

Maintain thorough medical documentation including case conferences, patient interactions, medication changes, re-certifications, progress notes, and care plan updates. Prepare visit reports and collaborate with other disciplines to ensure optimal patient care.
Required Experience and Skills

Nursing diploma, associate, or bachelor’s degree.

Preferred minimum of one year nursing experience, especially in medical-surgical, ICU, ER, or acute care settings.

Home health experience is a plus.

Current, unrestricted Registered Nurse license.

Valid CPR certification.

Strong organizational, communication, and interpersonal skills.

Valid driver’s license, reliable transportation, and auto insurance.
Compensation
-
Pay per visit/unit: $45.00 – $63.00
-
Annual base salary: $70,500 – $96,900 (full-time, 40 hours/week)
Note: Pay varies based on location, skills, experience, education, and certifications.
