As a Home Health Registered Nurse at CenterWell Home Health, you’ll have the opportunity to make a meaningful difference by providing personalized, one-on-one care that supports patients in regaining their independence—all within the comfort of their own homes. Collaborating closely with a dedicated team of physicians and clinicians, you’ll develop and manage individualized care plans that promote recovery and help patients return to the lives they cherish.
Key Responsibilities:

Conduct admission assessments, case management, and follow-up skilled nursing visits for home health patients.

Provide ongoing nursing care and case management, following guidance from the Clinical Manager.

Collaborate with physicians to develop and update treatment plans based on patient assessments and physician orders.

Deliver hands-on nursing care, manage care plans, and educate patients under Clinical Manager supervision.

Coordinate care with a multidisciplinary team, including Physical Therapists, Occupational Therapists, Home Health Aides, and other external providers.

Continuously communicate patient progress and care updates to physicians and Clinical Managers.

Implement patient care plans with active involvement from patients and their families to achieve optimal health outcomes.

Supervise and guide Home Health Aides, Certified Home Health Aides, and Personal Care Workers, ensuring accurate documentation and care aligned with the current care plan.

Manage patient discharges in consultation with physicians and Clinical Managers, completing all necessary clinical documentation.

Prepare comprehensive medical documentation, including visit reports, medication updates, care plan changes, and coordination with other healthcare professionals.
Required Qualifications & Skills:

Diploma, Associate, or Bachelor Degree in Nursing.

Minimum of one year of nursing experience preferred.

Experience in medical-surgical, ICU, ER, or acute care settings is advantageous.

Home Health experience is a plus but not required.

Current, unrestricted Registered Nurse license.

Valid CPR certification.

Strong organizational and communication skills.

Valid driver’s license, auto insurance, and reliable transportation.
Compensation:

Pay range: $49.00 – $69.00 per visit/unit

Annual base salary range: $77,600 – $106,600 (based on full-time 40 hours/week schedule)
