As a Home Health Registered Nurse at CenterWell Home Health, you’ll have the opportunity to make a meaningful difference by providing personalized, one-on-one care that supports patients in regaining their independence—all within the comfort of their own homes. Collaborating closely with a dedicated team of physicians and clinicians, you’ll develop and manage individualized care plans that promote recovery and help patients return to the lives they cherish.

Key Responsibilities:

  • Conduct admission assessments, case management, and follow-up skilled nursing visits for home health patients.

  • Provide ongoing nursing care and case management, following guidance from the Clinical Manager.

  • Collaborate with physicians to develop and update treatment plans based on patient assessments and physician orders.

  • Deliver hands-on nursing care, manage care plans, and educate patients under Clinical Manager supervision.

  • Coordinate care with a multidisciplinary team, including Physical Therapists, Occupational Therapists, Home Health Aides, and other external providers.

  • Continuously communicate patient progress and care updates to physicians and Clinical Managers.

  • Implement patient care plans with active involvement from patients and their families to achieve optimal health outcomes.

  • Supervise and guide Home Health Aides, Certified Home Health Aides, and Personal Care Workers, ensuring accurate documentation and care aligned with the current care plan.

  • Manage patient discharges in consultation with physicians and Clinical Managers, completing all necessary clinical documentation.

  • Prepare comprehensive medical documentation, including visit reports, medication updates, care plan changes, and coordination with other healthcare professionals.

Required Qualifications & Skills:

  • Diploma, Associate, or Bachelor Degree in Nursing.

  • Minimum of one year of nursing experience preferred.

  • Experience in medical-surgical, ICU, ER, or acute care settings is advantageous.

  • Home Health experience is a plus but not required.

  • Current, unrestricted Registered Nurse license.

  • Valid CPR certification.

  • Strong organizational and communication skills.

  • Valid driver’s license, auto insurance, and reliable transportation.

Compensation:

  • Pay range: $49.00 – $69.00 per visit/unit

  • Annual base salary range: $77,600 – $106,600 (based on full-time 40 hours/week schedule)
