Position Summary: Registered Nurse – Home Health
We are seeking an experienced and motivated Registered Nurse (RN) to join our home health team. This position involves providing skilled nursing care and community health education to patients within their homes. The RN is responsible for the coordination, planning, and delivery of high-quality care in accordance with patient needs and agency standards.

You will use your clinical knowledge to assess patients, develop individualized care plans, and ensure optimal outcomes—all while working independently and drawing upon community resources to support care delivery.

  • Boerne, TX

  • Bandera, TX

  • Surrounding communities


Key Responsibilities:

  • Plan, implement, and evaluate individualized care plans for patients receiving home health services

  • Deliver comprehensive nursing care including wound care, medication management, and health education

  • Supervise Licensed Vocational Nurses (LVNs) and Nurse Aides/Assistants as needed

  • Perform assessments and document patient progress in compliance with OASIS and agency protocols

  • Communicate effectively with patients, families, physicians, and interdisciplinary care teams

  • Ensure compliance with all applicable state and federal guidelines


Qualifications:

  • Active and unrestricted Registered Nurse (RN) license in the state of Texas

  • Bachelor’s Degree in Nursing (BSN) from an accredited institution is preferred

  • 1–2 years of recent acute care experience in an institutional setting

  • At least 1 year of home health care experience, preferred

  • Prior supervisory experience of LVNs or home health aides is a plus

  • Valid CPR certification (must include hands-on practical skills evaluation)

  • Familiarity with OASIS documentation requirements, preferred

  • Strong assessment, communication, problem-solving, and basic math skills

  • Reliable transportation and willingness to travel within the coverage area


Compensation:

  • Base Salary: $78,000 per year (Employer estimate)
