We are currently seeking a compassionate and skilled Registered Nurse (RN) to join our Private Home Care team. Key Responsibilities

Conduct thorough patient assessments and monitor health conditions on an ongoing basis

Assist with or perform diagnostic evaluations and contribute to individualized treatment planning

Administer medications and perform treatments as prescribed by healthcare providers

Collaborate with physicians, caregivers, and multidisciplinary team members to ensure coordinated care

Maintain accurate and up-to-date medical documentation and patient records

Observe, monitor, and report changes in patient condition promptly

Educate patients and families on health maintenance, prevention strategies, and care techniques

Follow individualized non-medical and medical care plans as outlined by the supervising provider or care team
Qualifications

Current, valid RN license in the state of Georgia

Prior experience in one or more of the following areas is a plus:

Infectious disease care

Medical office/clinic setting

Minimum Data Set (MDS) documentation

Solid understanding of nursing best practices and clinical standards

Strong interpersonal, communication, and organizational skills

Ability to work independently and manage time effectively in a home care environment
