Key Responsibilities

Engage with patients to navigate healthcare needs and communicate effectively with medical staff

Collaborate with clinical teams to manage a targeted population and improve health outcomes

Document patients’ medical history and issues accurately in the electronic health record (EHR)

Assist patients with referrals and connect them to community-based resources as needed

Meet Key Performance Indicators (KPIs) to enhance patient health outcomes and reduce medical spending
Required Qualifications

Certified or Registered Medical Assistant OR Certified EMT with 2+ years of experience

Strong interpersonal skills with empathy and professionalism

Proficiency in Microsoft Office, including Outlook, Word, and Excel

Ability to work independently under supervision of licensed professionals

Reliable transportation and a valid state driver’s license for occasional local travel
Preferred Qualifications

Experience with EHR systems such as eClinicalWorks, EPIC, Athena, or Cerner

Knowledge of motivational interviewing and behavioral change techniques
