Guidehealth is seeking a Healthguide to support patients and their primary care physicians by improving communication, navigating care, and helping achieve better health outcomes. This remote role involves collaborating with clinical teams, managing a targeted patient population, and assisting with care coordination and referrals. Candidates must live within 60 miles of Columbus, GA.
Key Responsibilities

Engage patients remotely through calls, texts, emails, and portal messages to schedule care and address health concerns

Collaborate with primary care staff and Guidehealth Case Management team to manage a targeted patient population

Assist patients in closing care gaps and achieving better health outcomes through high-impact care plans

Escalate medical issues to Registered Nurses, primary care physicians, or Guidehealth leadership as appropriate

Accurately document patients’ medical history and health issues in the electronic health record (EHR)

Support referral and prior authorization processes for specialty care and community-based resources

Build and maintain professional relationships with assigned medical practices and internal teams

Meet assigned Key Performance Indicators (KPIs) to optimize patient outcomes and reduce medical spending

Travel as needed (10-15%) to clinical sites in the Columbus, GA area for patient engagement and retinal exams (training provided)

Adhere to all security and HIPAA policies to protect patient information and Guidehealth intellectual property
Required Qualifications
