Guidehealth is seeking a Healthguide to support patients and their primary care physicians by improving communication, navigating care, and helping achieve better health outcomes. This remote role involves collaborating with clinical teams, managing a targeted patient population, and assisting with care coordination and referrals. Candidates must live within 60 miles of Columbus, GA.


Key Responsibilities

  • Engage patients remotely through calls, texts, emails, and portal messages to schedule care and address health concerns

  • Collaborate with primary care staff and Guidehealth Case Management team to manage a targeted patient population

  • Assist patients in closing care gaps and achieving better health outcomes through high-impact care plans

  • Escalate medical issues to Registered Nurses, primary care physicians, or Guidehealth leadership as appropriate

  • Accurately document patients’ medical history and health issues in the electronic health record (EHR)

  • Support referral and prior authorization processes for specialty care and community-based resources

  • Build and maintain professional relationships with assigned medical practices and internal teams

  • Meet assigned Key Performance Indicators (KPIs) to optimize patient outcomes and reduce medical spending

  • Travel as needed (10-15%) to clinical sites in the Columbus, GA area for patient engagement and retinal exams (training provided)

  • Adhere to all security and HIPAA policies to protect patient information and Guidehealth intellectual property


Required Qualifications

  • Residence within 60 miles of Columbus, GA

  • Strong communication skills with empathy and professionalism

  • Ability to work independently while collaborating effectively with remote teams

  • Familiarity with EHR systems and healthcare workflows

  • Reliable internet connectivity and ability to travel locally as needed


Preferred Qualifications

  • Prior experience in patient care, care coordination, or case management

  • Knowledge of referral processes, prior authorizations, and community-based health resources

  • Experience working with high-risk patient populations
