Coder IV Job Responsibilities
In this Coder IV role, you’ll apply expert knowledge of ICD-10-CM, ICD-10-PCS, and CPT coding systems to maintain compliance and data accuracy.
Core Duties
- Assign accurate codes for diagnoses, procedures, and treatments in inpatient and outpatient settings.
- Review provider documentation to confirm principal diagnoses, co-morbidities, and surgical details.
- Utilize MS-DRG and APC reimbursement principles for optimal data integrity.
- Clarify incomplete or ambiguous documentation through established query processes.
- Identify and report Hospital-Acquired Conditions (HAC) and Patient Safety Indicators (PSI).
- Collaborate with the Clinical Documentation Improvement team to resolve DRG mismatches.
- Maintain compliance with AHIMA’s Standards of Ethical Coding and official guidelines.
- Participate in department meetings, training, and coding audits to ensure continuous quality.

Required Skills & Experience
- High School Diploma or equivalent; completion of AHIMA or AAPC-accredited certification program required.
- Active coding credential (RHIA, RHIT, CCS, CCS-P, CPC, or CPC-H).
- Minimum 3 years of inpatient coding experience in an acute care hospital setting.
- Proficiency with ICD-10 coding, EMR systems, and MS Word.
- Ability to meet coding productivity and accuracy standards.
- Strong analytical, organizational, and communication skills.
Preferred Qualifications
- Experience using OptumCAC or Cemer encoder systems.
- Intermediate-level proficiency in Microsoft Excel.
- Experience with charge validation and observation calculations.
