Position Summary
We are currently seeking a Registered Nurse (RN) to join our team as a Care Coordinator. This role focuses on providing compassionate, patient-centered care management through assessment, planning, coordination, and evaluation of services. This is a remote, community-based position that requires flexibility to meet patients where they are—whether at home, in clinics, or other care settings.
Key Responsibilities

Conduct comprehensive assessments to identify patient needs and develop individualized care plans

Coordinate access to medical, behavioral health, and community resources

Assess and support mental health and substance use disorder treatment needs

Perform home visits and outreach in various settings to provide patient education and care planning

Continuously monitor and adjust care plans to ensure optimal outcomes

Partner with community health workers to address social determinants of health such as housing, transportation, and food insecurity

Advocate for patients in collaboration with community partners

Requirements

Registered Nurse (RN) license (Required)

Minimum 2 years of RN case management experience

Minimum 5 years of overall nursing experience

Strong knowledge of electronic medical records (EMR) and computer proficiency

Valid Washington State driver’s license and auto insurance
Preferred Qualifications

Bachelor’s degree in Nursing (BSN)

Certified Case Manager (CCM)

Experience in home health, public health, or care coordination
