Position Summary
As a Remote Case Manager, you will serve as a dedicated healthcare professional, advocating for and coordinating care for customers, families, and caregivers throughout their healthcare journey. You will collaborate with internal and external partners to ensure personalized, high-quality care, while supporting Evernorth’s commitment to delivering whole-person health solutions. This role requires an active, unencumbered RN license and at least two years of direct patient care experience.
Key Responsibilities

Partner with customers to develop and document personalized care plans using the medical management system.

Set and support customer goals through motivational interviewing and shared decision-making techniques.

Coordinate care across providers, caregivers, internal teams (nutritionists, pharmacists, behavioral clinicians, medical directors), and external healthcare resources.

Engage with customers via phone, email, chat, and text, empowering them to navigate their healthcare needs.

Track and report daily activity volume and outcomes in accordance with performance standards.

Prioritize tasks to meet business goals and ensure high-quality service delivery.

Maintain clinical skills and case management knowledge through ongoing education and training.

Collaborate with cross-functional partners to improve health outcomes and customer experience.

Utilize multiple platforms, tools, and systems efficiently to manage caseloads.

Adhere to URAC case management standards and certification requirements within four years of hire.

Required Qualifications

Active, unrestricted Registered Nurse (RN) license in a U.S. state or territory.

Minimum two (2) years of full-time direct patient care experience as an RN.

Strong clinical judgment and critical thinking skills.

Excellent communication skills (verbal and written).

Proficient with Microsoft Office (Word, Excel, Outlook) and internet-based research.

Ability to manage multiple complex cases in a dynamic environment.

Access to a stable, high-speed internet connection (minimum 10 Mbps download / 5 Mbps upload).
Preferred Qualifications

Experience with NICU or neonatal populations.

URAC-recognized case management certification (or willingness to obtain within 4 years).

Demonstrated sensitivity to culturally diverse situations and populations.
Compensation and Benefits

Base Salary: $76,600 – $127,600 annually (commensurate with experience, role location, and internal equity).
