Position Summary

As a Remote Case Manager, you will serve as a dedicated healthcare professional, advocating for and coordinating care for customers, families, and caregivers throughout their healthcare journey. You will collaborate with internal and external partners to ensure personalized, high-quality care, while supporting Evernorth’s commitment to delivering whole-person health solutions. This role requires an active, unencumbered RN license and at least two years of direct patient care experience.


Key Responsibilities

  • Partner with customers to develop and document personalized care plans using the medical management system.

  • Set and support customer goals through motivational interviewing and shared decision-making techniques.

  • Coordinate care across providers, caregivers, internal teams (nutritionists, pharmacists, behavioral clinicians, medical directors), and external healthcare resources.

  • Engage with customers via phone, email, chat, and text, empowering them to navigate their healthcare needs.

  • Track and report daily activity volume and outcomes in accordance with performance standards.

  • Prioritize tasks to meet business goals and ensure high-quality service delivery.

  • Maintain clinical skills and case management knowledge through ongoing education and training.

  • Collaborate with cross-functional partners to improve health outcomes and customer experience.

  • Utilize multiple platforms, tools, and systems efficiently to manage caseloads.

  • Adhere to URAC case management standards and certification requirements within four years of hire.

Required Qualifications

  • Active, unrestricted Registered Nurse (RN) license in a U.S. state or territory.

  • Minimum two (2) years of full-time direct patient care experience as an RN.

  • Strong clinical judgment and critical thinking skills.

  • Excellent communication skills (verbal and written).

  • Proficient with Microsoft Office (Word, Excel, Outlook) and internet-based research.

  • Ability to manage multiple complex cases in a dynamic environment.

  • Access to a stable, high-speed internet connection (minimum 10 Mbps download / 5 Mbps upload).


Preferred Qualifications

  • Experience with NICU or neonatal populations.

  • URAC-recognized case management certification (or willingness to obtain within 4 years).

  • Demonstrated sensitivity to culturally diverse situations and populations.


Compensation and Benefits

  • Base Salary: $76,600 – $127,600 annually (commensurate with experience, role location, and internal equity).
