Key Responsibilities

  • Conduct and delegate initial and ongoing patient assessments within your scope of practice.

  • Complete Start of Care, Resumption of Care, Recertification, and Discharge OASIS visits; provide routine care as needed.

  • Develop and implement individualized care plans that emphasize prevention, education, and wellness.

  • Coordinate resources and care delivery to ensure patient goals are met effectively.

  • Regularly reassess patient progress and update care plans as necessary.

  • Ensure timely communication among team members, physicians, administrative staff, and external case managers.

  • Match patients with appropriately skilled caregivers to meet care needs.

  • Participate in special projects and perform other related duties as assigned.

Qualifications

  • Bachelor of Science in Nursing (BSN) preferred.

  • Minimum of 2 years of clinical home care experience, with at least 1 year in a clinical management role preferred.

  • Current and valid RN license and CPR certification.

  • Strong knowledge of federal and state home health regulations, PDGM structure, and clinical best practices.

  • Exceptional communication, interpersonal, organizational, and problem-solving skills.

  • Must be fluent in English with reliable attendance.

  • Valid driver’s license, auto insurance, and dependable transportation are required.

Work Schedule

  • Full-time, 40 hours per week

Compensation Range

  • Estimated annual salary: $70,500 – $96,900

  • Actual pay may vary based on geographic location, experience, education, and qualifications.
