Key Responsibilities

Conduct and delegate initial and ongoing patient assessments within your scope of practice.

Complete Start of Care, Resumption of Care, Recertification, and Discharge OASIS visits; provide routine care as needed.

Develop and implement individualized care plans that emphasize prevention, education, and wellness.

Coordinate resources and care delivery to ensure patient goals are met effectively.

Regularly reassess patient progress and update care plans as necessary.

Ensure timely communication among team members, physicians, administrative staff, and external case managers.

Match patients with appropriately skilled caregivers to meet care needs.

Participate in special projects and perform other related duties as assigned.
Qualifications

Bachelor of Science in Nursing (BSN) preferred.

Minimum of 2 years of clinical home care experience, with at least 1 year in a clinical management role preferred.

Current and valid RN license and CPR certification.

Strong knowledge of federal and state home health regulations, PDGM structure, and clinical best practices.

Exceptional communication, interpersonal, organizational, and problem-solving skills.

Must be fluent in English with reliable attendance.

Valid driver’s license, auto insurance, and dependable transportation are required.
Work Schedule

Full-time, 40 hours per week
Compensation Range

Estimated annual salary: $70,500 – $96,900

Actual pay may vary based on geographic location, experience, education, and qualifications.
