Luthas Health Care, a dynamic and expanding home health agency, is currently seeking compassionate and experienced Registered Nurses (RNs) to perform in-home patient assessments across Plymouth and Barnstable counties. This per diem opportunity offers exceptional schedule flexibility, making it ideal for nurses looking to supplement their income, enjoy greater autonomy, or explore independent, community-based work.
As a valued member of our clinical team, you will play a critical role in delivering comprehensive, high-quality, and patient-centered care within clients’ homes. Your responsibilities will include conducting initial and follow-up assessments, formulating individualized care plans, and collaborating closely with an interdisciplinary team to ensure optimal health outcomes for patients with diverse medical needs.
Key Responsibilities

Conduct in-home nursing assessments to evaluate patient needs and determine eligibility for services

Develop, implement, and revise individualized plans of care in coordination with physicians and agency care teams

Administer treatments including medication management, injections, and wound care

Monitor and record vital signs, medical symptoms, treatment responses, and overall patient progress

Educate clients and caregivers on topics such as chronic disease management, medications, safety, and wellness

Coordinate follow-up care and services with interdisciplinary team members (e.g., therapists, HHAs, physicians)

Supervise and evaluate home health aides to ensure adherence to care plans and quality standards

Maintain accurate and timely documentation in accordance with federal, state, and agency guidelines
-
Provide specialized care for clients requiring durable medical equipment (DME), memory care, or support for developmental disabilities

Participate in quality assurance initiatives and promote compliance with all applicable health regulations
Qualifications

Current and active Registered Nurse (RN) license in the state of Massachusetts
-
Minimum 1 year of clinical nursing experience (home health experience preferred)

Strong skills in clinical assessment, critical thinking, and electronic documentation

Ability to work independently and provide high-quality care in clients’ homes

Experience with chronic conditions, memory support needs, and developmental disabilities

Reliable vehicle and willingness to travel within the assigned service area

Proficiency with Electronic Medical Records (EMR) and mobile documentation tools

Excellent interpersonal, communication, and organizational skills

Compensation & Benefits

Pay Range: $54.91 – $60.63 per hour (based on visit completion and job type)

Per Diem Rate: $90 per completed assessment visit
