Key Responsibilities:

Conduct comprehensive assessments and create individualized care/service plans for long-term care members.

Monitor and manage member conditions, identify changes, and adjust care plans as needed.

Coordinate care with members, families/caregivers, providers, and community resources to ensure appropriate services are delivered.

Educate members and families on conditions, benefits, treatment options, and available support services.

Facilitate service authorizations and collaborate with healthcare providers and specialists to implement treatment strategies.

Complete documentation in compliance with federal, state, and clinical guidelines.

Perform home and community visits as required to assess needs and deliver integrated care.

Participate in quality improvement initiatives and support new clinical team members through preceptorship and onboarding support.

Serve as a liaison and advocate for members, ensuring continuity of care and service satisfaction.
Qualifications:
Education & Experience:
-
Graduate of an accredited nursing program or bachelor’s degree with 4–6 years of relevant clinical experience.

Bachelor’s degree in Nursing (BSN) preferred.
Licensure/Certification:

Active and unrestricted RN license or NP license in Texas or a compact state.

Resource Utilization Group (RUG) certification required within 90 days of hire (specific to Superior HealthPlan).
Preferred Background:

Experience working in long-term care, case management, or acute care settings.

Familiarity with managed care organizations and interdisciplinary collaboration.
Compensation & Benefits:

Hourly Pay Range: $35.49 – $63.79 (based on experience, education, and qualifications)

Median Estimate: $49.64/hr (based on McAllen, TX market)
