Key Responsibilities:

  • Conduct comprehensive assessments and create individualized care/service plans for long-term care members.

  • Monitor and manage member conditions, identify changes, and adjust care plans as needed.

  • Coordinate care with members, families/caregivers, providers, and community resources to ensure appropriate services are delivered.

  • Educate members and families on conditions, benefits, treatment options, and available support services.

  • Facilitate service authorizations and collaborate with healthcare providers and specialists to implement treatment strategies.

  • Complete documentation in compliance with federal, state, and clinical guidelines.

  • Perform home and community visits as required to assess needs and deliver integrated care.

  • Participate in quality improvement initiatives and support new clinical team members through preceptorship and onboarding support.

  • Serve as a liaison and advocate for members, ensuring continuity of care and service satisfaction.


Qualifications:

Education & Experience:

  • Graduate of an accredited nursing program or bachelor’s degree with 4–6 years of relevant clinical experience.

  • Bachelor’s degree in Nursing (BSN) preferred.

Licensure/Certification:

  • Active and unrestricted RN license or NP license in Texas or a compact state.

  • Resource Utilization Group (RUG) certification required within 90 days of hire (specific to Superior HealthPlan).

Preferred Background:

  • Experience working in long-term care, case management, or acute care settings.

  • Familiarity with managed care organizations and interdisciplinary collaboration.


Compensation & Benefits:

  • Hourly Pay Range: $35.49 – $63.79 (based on experience, education, and qualifications)

  • Median Estimate: $49.64/hr (based on McAllen, TX market)
