License, Certification, Education or Experience:
REQUIRED:
- Current Washington State OT license.
- Must obtain an NPI number within 30 days of hire.
- Recent applicable clinical experience in a medical/surgical, rehab, or home health setting.
- Current Washington State Driver’s license and proof of insurance.
- Current Healthcare Provider BLS certification.
- Reliable transportation.
DESIRED:
