Job Overview

The Social Worker – PACE role brings a psychosocial perspective to the interdisciplinary team, focusing on evaluation, assessment, care planning, and ongoing services for PACE participants. Responsibilities include:

  • Conducting individual and family assessments, education, and counseling
  • Managing behavioral health needs, case management, and advocacy
  • Coordinating resources to enhance participant care and quality of life
  • Collaborating with the Interdisciplinary Team to optimize patient outcomes

Corporate Responsibilities

This role supports four key areas within the PACE Program:

  1. Coverage Support: Fill in for unplanned MSW absences at PACE centers in Los Angeles and Orange Counties, ensuring compliance with assessments, care planning, and IDT participation.
  2. Onboarding & Training: Assist the Social Work Manager in planning and executing new employee orientation and onboarding for social workers.
  3. Data & Analytics: Collect and analyze data, prepare reports, and present findings to PACE leadership to enhance program effectiveness.
  4. Process Improvement & Committees: Actively participate in process improvement initiatives and committee meetings to improve psychosocial and behavioral health service delivery for PACE participants.

Minimum Requirements

  • Master’s Degree in Social Work (MSW) from an accredited institution
  • Experience in case management, social advocacy, or mental health preferred
  • Bilingual in Spanish/English, Chinese/English, or another second language strongly preferred
  • Current BLS certification required

Compensation

Salary Range: $69,420.62 – $86,775.78 annually
