Position Summary
Reporting directly to the Director of Pharmacy, the Staff Pharmacist I is responsible for the efficient and compliant operation of pharmacy services. This includes the preparation, verification, dispensing, and distribution of medications in accordance with established policies, procedures, and federal/state regulations. The pharmacist will ensure a high standard of pharmaceutical care for hospice patients while promoting safety, efficiency, and accuracy.
Core Responsibilities
Clinical & Operational Duties

Interpret and verify medication orders and prescriptions to ensure accuracy, appropriateness, and compliance.

Evaluate drug therapies for efficacy, contraindications, interactions, duplications, and allergies.

Perform sterile compounding using proper aseptic techniques for parenteral admixtures.

Provide patient counseling on prescription and over-the-counter medications.

Monitor the use and efficacy of prescribed medications and consult with prescribers when needed.

Dispense medications accurately, including checking technician-prepared items such as IVs and oral/topical doses.
Technical & Administrative Functions

Input and update patient demographics and medication profiles in the system.

Ensure accurate maintenance of controlled substance inventory, including Schedule II logs.

Utilize and maintain pharmacy technologies, including EyeCon, Smart Cabinet, and other dispensing tools.

Package medications and prepare them for delivery to patients or facilities.

Perform pharmacy opening and closing duties and ensure readiness for daily operations.
Communication & Collaboration

Respond to inquiries from physicians, nurses, and patients with drug information, dosage forms, packaging sizes, and cost data.

Answer pharmacy lines promptly (within three rings) and handle communication in a professional, courteous manner.

Maintain organized files, records, and documentation for both clinical and regulatory purposes.

Actively contribute to interdisciplinary care and quality improvement initiatives within the hospice setting.
Compliance & Quality Assurance

Follow all departmental, state, and federal regulations regarding pharmaceutical practices.

Participate in quality improvement goals and adhere to environmental safety and infection control standards.

Stay up-to-date with relevant pharmacy practice laws and guidelines.
Qualifications
Education & Licensing

Bachelor’s or Doctorate in Pharmacy from an accredited college of pharmacy.

Current, unrestricted license as a Pharmacist in the State of Florida (required).

Consultant Pharmacist license in Florida or willingness to obtain (required).
Experience
-
Minimum of 2 years of experience in a retail and institutional pharmacy setting (preferred).

Demonstrated experience with aseptic compounding techniques.

Completion of the Staff Pharmacist I probationary period, if internal.
Additional Requirements

Must be available to work evening shifts (1:30 PM – 10:00 PM), Monday through Friday.

Ability to relocate to Jacksonville, FL before starting work (required).
