Requirements:

  • Experience: A minimum of one year of acute care experience with specialized training in chemotherapy is required.

  • Licensure: Must hold a current and valid Pharmacist License issued by the State of New Jersey.

  • Education: Completion of a Bachelor of Science degree in Pharmacy or graduation from an Accreditation Council for Pharmacy Education (ACPE)-accredited school of pharmacy, or an equivalent educational program.

Primary Responsibilities:

  • Medication Order Review and Dispensing:
    Carefully evaluates and dispenses physician-ordered medications, ensuring accuracy, appropriate dosing, and therapeutic suitability. Proactively identifies and resolves any ambiguities, omissions, or potential issues in medication orders by consulting with prescribers when necessary.

  • Drug Therapy Monitoring:
    Closely monitors patients’ medication regimens to assess for potential allergies, drug-drug and drug-food interactions, contraindications, duplications, and any signs of adverse drug reactions. Provides clinical recommendations as needed to optimize therapeutic outcomes and ensure patient safety.

  • Sterile Compounding and Admixture Preparation:
    Prepares sterile intravenous admixtures, including total parenteral nutrition (TPN) and chemotherapy agents, utilizing strict aseptic techniques in accordance with USP standards and facility protocols.

  • Unit Inspections and Medication Storage Compliance:
    Conducts routine inspections of assigned units to ensure that medication storage areas are clean, organized, and compliant with regulatory and institutional policies. Verifies the presence and proper condition of emergency drug supplies, removing and replacing expired or damaged items as necessary.
