Position Summary

As a Staff Pharmacist at MarinHealth, you will play a vital role in optimizing pharmaceutical care, ensuring safe and effective medication use, and contributing directly to patient outcomes. This role involves both clinical and distributive responsibilities, including participation in interdisciplinary care, pharmacotherapy evaluations, and medication consultations.

Depending on qualifications and interests, pharmacists may rotate through specialized areas such as Critical Care, Emergency Department, Transitions of Care, and Oncology Infusion Pharmacy. You’ll work collaboratively with physicians, nurses, and patients while overseeing the accurate dispensing and safe use of medications within the hospital setting.


Key Responsibilities

  • Deliver comprehensive clinical and distributive pharmacy services with a focus on patient-centered care.

  • Monitor medication therapies for appropriateness, dosing, efficacy, side effects, and clinical progress.

  • Review and intervene on unclear, inappropriate, or suboptimal medication orders.

  • Promote cost-effective, evidence-based medication use and uphold the hospital’s closed formulary.

  • Provide drug information and education to healthcare professionals, patients, and caregivers.

  • Support discharge planning through medication teaching and counseling.

  • Ensure accurate processing and timely distribution of medications.

  • Participate in continuous improvement, policy development, and drug utilization reviews.

  • Contribute to department projects, interdisciplinary teams, and educational efforts.

  • Respond to Code Blue emergencies and attend patient care meetings as needed.

  • Mentor pharmacy interns, residents, and new staff.


Qualifications

Education:

  • Required: Graduation from an accredited School of Pharmacy and eligibility for California pharmacist licensure.

  • Preferred: Doctor of Pharmacy (Pharm.D.) and/or completion of an ASHP-accredited residency program.

Experience:

  • Preferred: At least 2 years of experience in an acute care hospital setting.

Licensure:

  • Required: Active and current Registered Pharmacist license in the State of California.
