Staff Physical Therapist, Per Diem – Ambulatory Care Center / Outpatient

Opportunity: We seek a skilled Per Diem Staff Physical Therapist to evaluate patient needs and implement comprehensive rehabilitation programs in our Ambulatory Care Center / Outpatient setting.
Responsibilities:
- Conduct initial assessments and evaluations of physical therapy needs for assigned patients.
- Develop and implement effective treatment plans and protocols.
- Set achievable treatment goals in collaboration with patients and families.
- Provide patient and family education and training.
- Complete all required documentation accurately and timely.
- Participate in the clinical education of fieldwork students (if applicable).
- Maintain expected patient care productivity levels.
- Demonstrate ongoing proficiency in relevant skills.
- Perform other duties as assigned.
Minimum Qualifications:
- Bachelor’s Degree in Physical Therapy.
- Current New York State Physical Therapy License or temporary permit.
- Strong communication and interpersonal skills.
- BLS certification (American Heart Association) or Heartsaver CPR/AED (AHA) upon hire.
Preferred Qualifications:
- Master’s Degree in Physical Therapy or related field.
